Forms (to Sign Up, Report, Request and/or Complaint)


The Church Point Homeowners Association maintains a number of forms to allow home/lot owners to interact with the Board of Directors, Committees and Management Company. The City of Virginia Beach also has a number of online mechanisms to report issues to them. Below is a list of these forms/mechanisms organized by purpose.

Signing up for the Opt-in/Opt-out Notification System

  • To sign up for automated email notifications, please use the Opt-in/Opt-out Communications Signup Form. Instructions of how to sign up, change your notification preference or remove yourself from the list are on the same page.

Report an Issue

Depending on the type of issue, two different mechanisms are available. The first mechanism is to report Public Works issues to the City and the second mechanism is to report Law Enforcement issues to the Police Department:

Request Permission to Make an Architectural Change to your Property

File a Formal Complaint against the Board or Management Company

  • Complaints Procedure and Form – Fill out this paper form to file a written complaint regarding the action, inaction or decision by the Board, Managing Company or Association. Instructions are included in the form.
