The Church Point Homeowners Association maintains a number of forms to allow home/lot owners to interact with the Board of Directors, Committees and Management Company. The City of Virginia Beach also has a number of online mechanisms to report issues to them. Below is a list of these forms/mechanisms organized by purpose.
Signing up for the Opt-in/Opt-out Notification System

Report an Issue
Depending on the type of issue, two different mechanisms are available. The first mechanism is to report Public Works issues to the City and the second mechanism is to report Law Enforcement issues to the Police Department:
- Report Issues to the City of Virginia Beach – Read this page on how to report Public Works issues to the City of Virginia Beach related to our streets, sidewalks and/or street lights.
- Report Non-Emergency Law Enforcement Issues to the Virginia Beach Police Department – The Police Department provides an online mechanism to report issues like noise violations, neighborhood crime, destruction of property, traffic incidents, abandoned vehicles, embezzlement and identity theft.
The second mechanism is to report Church Point Homeowners Association specific issues:
- Report Landscaping Issue – Fill out this form to report an issue in the Church Point Common Areas. Your report will be submitted to the Landscaping Committee for action.
- Report General Issue – Fill out this online form to report a general (non-City and non-landscaping) issue to the Board of Directors.
Request Permission to Make an Architectural Change to your Property
- Application for Architectural Projects – Fill out this paper form if you plan to make external changes to your home. Instructions are included in the form.
File a Formal Complaint against the Board or Management Company
- Complaints Procedure and Form – Fill out this paper form to file a written complaint regarding the action, inaction or decision by the Board, Managing Company or Association. Instructions are included in the form.
